Name Based Background Check

I authorize Efficient Biometrics to submit a name-based criminal history record inquiry to the Illinois State Police (ISP) using the personal identifiers I provide (such as name, sex, race, and date of birth) for the purpose I have selected.

I understand that a name-based check is based solely on those identifiers and is not a fingerprint-based search. A name inquiry may return multiple possible matches ("hits"), or may not return a record that exists under an alias or a different date of birth. For a definitive result, a fingerprint-based check may be required.

I certify that the information I provide is true and accurate to the best of my knowledge, and that I am authorized to request this check for the purpose selected.

Illinois UCIA Notice

This service is provided under the Illinois Uniform Conviction Information Act (UCIA), 20 ILCS 2635. Any person who knowingly requests, obtains, or seeks to obtain conviction information under false pretenses, or who knowingly communicates or seeks to communicate conviction information to any agency or person except in accordance with the Act, or who knowingly falsifies conviction information, may be guilty of a criminal offense and may incur civil liability.

When a name-based check is requested for employment purposes, the applicant’s signature/consent is required and must be retained by the requesting employer for two years. The employer must provide the applicant a copy of any response received.

Privacy & Payment

The information I submit is used to process this request and is stored securely. Efficient Biometrics handles personal data in accordance with its Privacy Policy and Biometrics Retention and Destruction Policy, available at www.efficientbiometrics.com.

Payment is required at the time of the request to initiate processing. All transactions are final and non-refundable. Results, when available, are delivered to the email address provided.
